Blaylar Health is required by law to maintain the privacy of your protected health information, provide this notice, and follow the terms currently in effect.
How We May Use and Disclose Your Health Information
We may use and disclose your information for treatment, payment, healthcare operations, appointment reminders, treatment alternatives, business associate services, and other purposes allowed or required by law.
Uses and Disclosures Requiring Authorization
Uses not otherwise permitted by this notice or applicable law generally require your written authorization. You may revoke an authorization in writing, subject to legal and operational limitations.
Your Rights
- Right to inspect and receive a copy of information used to make decisions about your care.
- Right to request amendments to certain information.
- Right to request an accounting of certain disclosures.
- Right to request restrictions and confidential communications in certain circumstances.
- Right to obtain a paper copy of this notice.
Patient-Directed Disclosures to Apps
You may direct us to send your health information to an app you choose. If that app is neither a HIPAA covered entity nor a business associate, the copy it receives at your direction is no longer protected by HIPAA. Many consumer apps fall into this category. Other privacy and consumer protection laws may still apply, and the app's own policy describes its use, sharing, sale and disposal practices. Information retained in our clinical record remains subject to our HIPAA duties. Review the app's policy before authorizing access and our connected-app privacy notice for consent, device permissions, revocation and deletion requests. Revoking access does not recall copies already disclosed or require deletion of records we must lawfully retain.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with Blaylar Health or with the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.
